I. DAY, TIMES AND PLACE OF SEMINARS:
All seminars are held in your facilities. Two day in-house seminars can be held on two concurrent days including a Friday and Saturday (NOTE: Honorarium varies with days chosen).
One-day seminars may be held on any weekday.
FIRST DAY - 9:00 A.M. to 4:30 P.M. with Lunch break from 12:00 - 1:00 (Coffee Break at 10:15 A.M. and 2:45 P.M.)
SECOND DAY (No second day with a one-day seminar) - 8:30 A.M. to 3:00 P.M. with Lunch break from 11:30 - 12:30 (Coffee Break at 9:30 A.M. and 2:00 P.M.).
II. PROJECTED COSTS
ONE DAY SEMINAR - HONORARIUM
(Effective January 1, 2008)....................... $3,500 Plus All ExpensesBASIC HONORARIUM PACKAGE - TWO DAY SEMINAR
(Effective January 1, 2008)....................... $6,000 Two-day Seminars Include All Of The Following:
- Friday and Saturday seminar presentation (in continental USA).
- Gene Levines Air Fare from Gainesville, Florida regional airport to your areas regional airport, and return, Florida airport parking, lodging (number of applicable seminar days +1), meals and miscellaneous out-of-pocket expense.
- Gene Levines compensation for presenting the seminar.
- Free factory consulting visitation by Gene Levine before the seminar (full day, less his applicable travel time).
- Free use of GLAs "Supervisory Curriculum Development And Planning Questionnaires". Copies to be completed by all your attendees prior to the proceedings. Send completed forms to GLA for collating and analysis. The tabulated results of these questionnaires, plus the factory visitation by Gene Levine, will allow the tailoring of his presentation to your specific needs.
- If applicable, you will receive six (6) free course training manuals (except ASSETT©), applicable reference materials, handouts and six (6) Certificates of Completion.
- Free phone training support for attendees for one year.
- Up to eight (8) hours round trip portal to portal travel time (GLAs High Springs, Florida offices and your facilities).
When Applicable, In Addition To The Above Honorarium And Activities, Add The Following Dollar Amounts:
- For a two-day seminar, scheduling the presentation for two concurrent weekdays (Monday - Friday) add $1,500
- Each additional course training manual, applicable reference materials, handouts and Certificates of Completion over the six (6) provided and free toll-free phone support (for those attendees) for one year. (Stated list price of manual.)
- Each additional hour of round trip portal to portal travel time over the eight (8) allowed hours add $100.00
- Any required rental car, additional lodging and/or out-of-pocket expenses. Billed at cost.
- For the two-day seminar; for each additional man-day beyond the one-free day (prior to the seminar) that man-day will be billed at prevailing rate.

You Will Also Need:
- Blank 3 Ring binders for training manuals
- Attendees meals, coffee breaks, and miscellaneous supplies
- Required Audio Visual equipment (see list below)
- If a Time Study seminar, you will need to supply a CD containing video taped segments of about 10 various, simple factory jobs that attendees will time during the seminar. Each attendee will need a decimal minute stopwatch (100 increments to a minute – 60 seconds/minute not acceptable) and a suitable stopwatch holder. III. SOME REQUIRED AUDIO VISUAL EQUIPMENT (Varies By Presentation)
A. Media projector with laptop interface (or equivalent).
B. Wireless clip-on lavaliere mike (Only necessary for groups of more than 25 Attendees).
C. Chalkboard, or whiteboard and applicable chalk, whiteboard pens, eraser, extension cords, etc.
D. High stool, and some type of floor lectern and a 4 X 8 table to put speaker's passouts and other materials on.
E. Screen for media projector.
ADDITIONAL REQUIREMENTS:
Pencils and pads for each attendee.
Because of the seminars media lighting requirements, the seminars cannot be presented in brightly lit rooms where there are no means to dim or blackout the light. So, if the presentation room has windows they must have room-darkening drapes, blinds or shades.
IV. REQUIRED DEPOSIT AND FINAL REMUNERATION
To avoid any financial misunderstandings, We require a 50% Non-Refundable deposit of the applicable honorarium at confirmation time. This is required because we are prevented from accepting any other assignments due to your booking our time and should you cancel or change the dates we have lost billing income. The balance of the honorarium is paid at the conclusion of the seminar.
V. SEMINAR/WORKSHOP CURRICULUM DEVELOPMENT
We suggest that all you key people make a prioritized list of your training requirements and immediately send that list to GLA (we treat all correspondence very confidentially). You also might want to give your supervision the opportunity to help decide the workshops curriculum.
To help you better determine your training needs, we have a proven idea to help you. This idea takes the form of our "Supervisory Development Curriculum Planning Questionnaire". You can modify this document if you wish. We further suggest you explain to the questionnaire recipient(s) that their answers are confidential and that their signature is optional.
When you forward copies of the questionnaires to our offices we will use that information to prepare a presentation tailored to your needs.
